Missed appointments or cancellations without providing 24 hours notice are subject to a $25 fee on the first occurrence and then charged the full appointment price for any occurrence thereafter.
In order to make the most of your time with us, we ask that you read and complete the paperwork at home. This will give you time to thoroughly explain your answers because we value your concerns, your goals, and want to help you. The best way to do this is to have a complete and accurate history.
